In Type-2 diabetes common complications reported are nephropathy, retinopathy, neuropathy, coronary artery disease, peripheral vascular disease and increased risk of infections. The prevalence, morbidity and mortality from liver disease has been largely overlooked. Non Alcoholic Fatty Liver Disease (NAFLD) and Type 2 Diabetes Mellitus (T2DM) affect global health and can influence each other. Possible association of NAFLD in non obese type 2 diabetes with microalbuminuria, dyslipidemia and central obesity is poorly studied.
Aim: To find association of NAFLD with microalbuminuria, HbA1c, duration of diabetes, lipid profile, WC and WHR in non-obese type 2 diabetic patients.
This cross-sectional unicenter non interventional study was conducted in 75 non obese T2DM patients having NAFLD. They were performed ultrasound abdomen and ARFI (Acoustic Radiation Forced Imaging). Based on the results, they were divided into four Groups- Grade I, II, III fatty liver and NASH. Microalbuminuria was detected by spot urine albumin/creat ratio. Serum lipid profile and HbA1C was measured. Waist circumference, hip circumference and Waist Hip Ratio and BMI calculated.
75 non obese T2DM patients having NAFLD were enrolled of which 40 (53.3%) were males and 35 (46.7%) were females. Microalbuminuria was observed in 24 (32%) individuals. Severity of NAFLD directly correlated with severity of microalbuminuria , dyslipidemia and was found to be statistically significant. Severe NAFLD also correlated with duration and severity of glycemic control and was statistically significant. Central obesity was observed in more severe NAFLD.
ARFI can be used for the early detection of NAFLD. More severe NAFLD correlated with poor glycemic control, longer duration of diabetes, dyslipidemia and WHI.
