THE EFFECT OF RADIATION THERAPY IN PATIENTS WITH ACROMEGALY DEPENDING ON THE AGE OF PATIENTS

Objective : To evaluate the effectiveness of radiation therapy (RT) in patients with acromegaly, depending on the age of patients and the activity of pituitary adenomas.


Methods :

The subject of the study were 50 patients (women-36, men-14) with acromegaly who received gammatherapy in a total dose of 45-60Gr of 20-25fractions every other day. The duration of the disease was 15years on average.


In general, the age of patients ranged from 26 to 77years, on average-44.7±6.8years. By age, the patients were divided into 3 groups: the Iage group-29-44years-23patients (46%), the IIgroup-45-59year-16patients (32%), the IIIgroup-60-79years, 11patients (22%). All patients underwent hormonal (GH, IGF-1) studies.


Results :

The levels of hormones of GH and IGF-1 remained high in the I group in 22% of patients, and in group III in 9%. Remissions reached 64% in the III age group. The highest level of GH content occurred in the age range from45 to59years and was 77.5±9.68mMе/l, and from 26 to 44years it was 68.1±6.84mMе/l. After RT, this indicator in both age groups was almost equally suppressed, but not up to the norm.


The decrease in the GH level to normal was observed in Iage group in 8patients, and in the IIage group in 5patients it was 2.65±2.2mMе/l and 2.23±1.4mMе/l, respectively. In groupIII, in 11 patients before RT, the mean level of GH was 46.5±8.2mMе/l. After RT, the level of GH decreased in this way: not suppressed only in one patient and was 22.1±0.89mMе/l, in 3patients the level of GH was suppressed, but not up to the norm and amounted to6.7±0.7mMe/l, only 7patients were suppressed and amounted to 2.46±2.2mMe/l.


Discussion : As the results of the study showed, the use of radiation therapy leads to a significant decrease in the level of GH and IGF-1 in all age periods, and a particularly significant decrease was observed in age group III.


Conclusion : In all age periods, even in the period of the most activity pituitary adenoma-at the age of 26-44 years, RT is a rather effective method of treatment and gives positive results in most cases.

I am Issaeva Saodat Saydullaevna, born in the family of office workers on January 1, 1986 in the town of Sukok (Parkent), Tashkent Region, the Republic of Uzbekistan. From 1992 to 2001 I studied at the secondary school № 17 of Sukok district. In the same year I entered the Tashkent Medical Pediatric Institute. On graduation of the Institute in 2008 I qualified as "GP (General Practitioner) and Pedagogue".
From 2008 to 2012 I worked the Tashkent Medical College.
From 2012 to 2015 I had a course of magistracy at the Tashkent Medical Pediatric Institute. I underwent practical training in the following departments of the Center: Neuroendocrinology, Pediatric Endocrinology, Endocrinous Surgery, the Thyroidal Department, Diabetology, Diabetic Nephropathy, Hemodialysis, Diabetic Foot, Radionuclear Diagnostics.
I had a master's scientific work was for acromegaly. Topic title: Optimization of management of patients with acromegaly in the background radiation therapy.
On graduation from the magistracy I started my labor activities in the Republican Specialized Theoretical and Practical Medical Center of Endocrinology. And I'm currently studying in postgraduate school.
In 2010 I got married to Mr. Sh. Rasulov. We have a two daughters Yoldorova Feruza, Yoldarova Firdavsiya and a son Yoldorov Abdulvosiy.

I was born in 1965 in the city of Bukhara, family of workers. Father Yusuph Khalimov – is driver, mother – Solikha Khalimova is a housewife, who engaged 10 child rearing.
In 1982 I graduated middle school № 22 in the city of Bukhara with a gold medal. This year I entered the Tashkent Medical Institute. In 1988 had completed the institute and entered to internship in the speciality of neurology. 1991-1994 years worked under the supervision of academic N.M. Majidov as a junior researcher at the central laboratory of Tashkent Medical Institute. In 1994 completed PhD degree in neurology Implication of heliogeophysical factors in the development of cerebral stroke in Uzbekistan. In 1995, I worked as assistant in the department of neurology at the Tashkent Medical Institute and scientific secretary at the Academy of Sciences of Uzbekistan. Since 1996 under the supervision of academic Yo.Kh.Turakulov I entered the doctorates program at Endocrinology Research Institute. Since then my main scientific direction was dedicated to the reserve activities of the neuroendocrine glands, hypothalamic-pituitary system. 1996 -2002 years I worked as a senior researcher at the laboratory of neuroendocrinology.
In 1997, under the supervision of academic Yo.Kh.Turakulov were expanded Neuroendocrinology departments. Promoting charity and supporting the Embassy of Japan in Uzbekistan received modern medical equipment for the diagnosis and treatment of difficult to diagnose diseases of the neuroendocrine system. I was in the city of Prato (Italy) 2-week training course for working in this technique.
In 1997 was open a highly qualified Neuroendocrinology Department. And until today I am the scientific and practical leader of this department.
From January 1998 until June I worked on a doctorate in Europe in the Department of Neuroendocrinology, at the St Saint Bartholomew’s Hospital at to the University of London which is headed by a member of the Academy of the Kingdom of Great Britain, neyroendocrinologist, professor Ashley Barry Grossman. The main part of the doctoral thesis was conducted at this center which is devoted to the problems of diagnosis of pituitary adenoma, their immunogenetic basis and treatment problems.
In 2000 I defended my doctoral dissertation which called «Pituitary adenomas, modern diagnostic and treatment methods».
Since 2002, collaboration has been established among the Neuroendocrinology Laboratory and the Department of Neuroendocrinology at the University of London and I together with well-known neuroendocrinologist of Europe and Great Britain prof. A.B.Grossman and neurosurgeon prof. M. Pauell conducts scientific researches and practical master classes to 1-2 times a year.
Since 2004, microscopic transphenoidal pituitary operations have been performed in our countries. In 2010, in the center of endocrinology was open a highly qualified neurosurgery department and from this year, the center regularly conducts microscopic and endoscopic operations of the pituitary adenoma..
2003 - 2006 I was secretary of the Endocrinology Association of Uzbekistan, since 2007 editor of medical journal «Diabetes in Uzbekistan». Since 2009 until today chairman of the Appraisal Committee in specialty of «Endocrinology» in Uzbekistan. Each year I participat in various congresses in Uzbekistan and in the World. I am a member of Neuroendocrinology Associations. We received considerable number of neuroendocrinology grants on the state level and conducted research projects on them.
