26 - RELATIONSHIP BETWEEN BONE AND CARDIOVASCULAR HEALTH : BONE MARKERS AND CARDIOVASCULAR RISK FACTORS IN ADULT NIGERIANS

Objective : Osteoporosis and cardiovascular disease are of global public health concerns. Both disease states have been linked through common risks factors and biological mechanisms. There is dearth need to report this inter-relationship in Nigerians. Therefore, this study sought to determine inter-relationship between Bone markers and some metabolic risk factors. This study was to evaluate association between bone markers, and some cardiovascular risk factors in healthy adult Nigerian population.


Methods : This was a prospective study consisting of forty (40) healthy participants of less than 50years, selected by random systematic sampling. Clinical data were obtained through interviewer based questionnaire. Anthropometric indices were measured. Blood pressure was determined. Fasting blood sample were analysed for Osteocalcin(OC), Total Alkaline phosphatatase (ALK-P), 25-hydroxyvitamin–D(25-OHVitD), Parathyroid hormone (PTH), Calcium (CA), Fasting Blood Glucose (FBS) . 2hours post-prandial blood analysed for glucose (2HPP). 24 hours urinary calcium excretion (CaE) determined


Results : The mean age of studied was 34.1yrs (7.8), BMI =26.31kg/m2 (4.02), waist circumference (WC) =78.6cm (17.1), SBP =112.4 (12.96)mmHg, DBP =76.0 (7.09)mmHg, FBS =84.9 (12.64)mg/dl, 2HPP =106.0mg/dl (14.16), 25-OHVIT-D =53.53ng/dl (15.45). OC directly correlated with ALK-P (r = 0.4 p=0.006) but, not to CaE (>0.05). Serum OC inversely correlated with BMI, WC, FBS, p<0.05 and CaE showed inverse correlation to 25-OH vit D (p=0.041) and DBP (p=0.028), FBS p=0.015), a trend with SBP(0.086), ALK-P negatively correlated with FBS= r -0.3, p=0.04, 2HPP=r -0.65, p=0.000. There was no correlation between OC,ALK-P and CaE.


Discussion : Our results showed positive correlation between bone markers osteocalcin and Alkaline phosphatase (markers of osteoblastic activity), however, not with urinary calcium excretion(marker of osteoclastic activity). Serum Osteocalcin/Alkaline phosphatase associated with fat and glycemia indices (BMI and WC, FBS ,2HPP), and Calcium excretion with reduced vitamin D, glycemia, blood pressure measures (<0.05). Some studies had confirmed inverse relationship between osteocalcin, and glycemia indices ,atherosclerosis. But there are no reports on relationship between Alkaline phosphatase, calcium excretion, and metabolic indices. Our research showed deranged bone markers may be an indicator of cardiovascular disease.


Conclusion : From our study, there  is relationship between bone markers and cardiovascular risk factors in Nigerians. Therefore, we suggest that patients with cardiovascular disease will benefit from bone health screening or vice versa.
