12 - STRAIN ELASTOGRAPHY + 4D ULTRASOUND - DIAGNOSTIC IMPACT IN FOLLICULAR NEOPLASIA CASES

Objective : Follicular neoplasia is still a diagnostic challenge regarding preoperative diagnostic. The main aim of our study is to evaluate the performance of advanced ultrasound techniques in the preoperative diagnostic of cases evaluated as Bethesda IV cases after FNAC.


Methods : We evaluated 28 Bethesda IV cases (28 nodules) using conventional 2B ultrasound, real time Doppler evaluation (4D) respectively real time elastography, using a linear multifrequency probe and a linear volumetric probe (Hitachi Prerius Machine, Hitachi Inc, Japan). All the nodules were classified using a risk stratification model using seven conventional US characteristics, two 4D characteristics and color map RTE aspect. The results were compared with the pathology results, considered as golden standard diagnosis.


Results :

The prevalence of malignant nodules was 39.28% (11/28 cases). Conventional risk classification generated: 7 low risk cases, 12 intermediate risk and 9 high-risk cases. All 7/9 high-risk cases where FTC cases, 3/12 intermediate risk cases where FTC cases and just 1/7 low risk cases was FTC.  2 high-risk cases were simple follicular adenomas, 9/12 intermediate risk were benign follicular neoplasia, and almost all low risk cases were benign lesions. The high-risk classification appeared due to high stiffness of elastography or thyroid capsule rupture on 4D evaluation. Intranodular vascularization was present in both benign and malignant follicular neoplasia.

Conclusion : Advanced ultrasound techniques do add a preoperative diagnostic performance, but still a lot of cases are misdiagnosed preoperatively.
