30 - A COMPARATIVE STUDY OF SPUTUM CONVERSION RATE AMONG DIABETIC AND NON-DIABETIC PATIENTS WITH NEWLY DIAGNOSED SPUTUM SMEAR POSITIVE PULMONARY TUBERCULOSIS

Objective :


THE INCIDENCE OF TB IN INDIA IS 2.8 MILLION. WITH THE RAPID URBANISATION AND POPULATION EXPLOSION, 7.8 % OF INDIANS ARE BURDENED BY DIABETES MELLITUS WITH AN EQUAL PREVALENCE AMONG MALES (7.9 %) AND FEMALES (7.5 %)
ONE UNTREATED SPUTUM SMEAR POSITIVE PULMONARY TB PATIENT IS LIKELY TO INFECT 10-15 PERSONS ANUALLY. 80 – 90% OF PATIENTS WILL UNDERGO SMEAR CONVERSION WITH IN 2-3 MONTHS IF NOT ASSOCIATED WITH FACTORS LIKE UNCONTROLLED HYPERGLYCEMIA / DIABETES MELLITUS.
THIS IS OF SIGNIFICANCE OWING TO THE NEED OF INFECTION CONTROL MEASURES TILL NON-INFECTIOUSNESS IS DEMONSTRATED BY CULTURE/ SMEAR CONVERSION


Methods :

A PROSPECTIVE OBSERVATIONAL STUDY DESIGN WAS USED WITH SUB-GROUPS OF TB PATIENTS, ONE WITH KNOWN DIABETIC(N=50) STATUS AND THE OTHER WITHOUT DM(N=50).TB PATIENTS REGISTERED IN RNTCP WERE CONSIDERED IN THE STUDY. DIABETES CONTROL WAS ASSESSED BY HBA1C VALUES AT DIAGNOSIS OF TB AND AFTER TWO MONTHS OF ATT. DATA ON FOLLOW-UP SPUTUM EXAMINATIONS WAS COLLECTED.


Results :

 THE INCIDENCE OF TUBERCULOSIS WAS HIGHER AMONG MALES IRRESPECTIVE OF DIABETIC STATUS.
 3+ SPUTUM BACILLARY LOAD WAS SEEN IN 10 % NON DIABETICS AND 40 % DIABETICS.
 94 % OF NON DIABETICS SHOWED SPUTUM CONVERSION WHILE ONLY 82 % OF DIABETICS SHOWED SPUTUM SMEAR NEGATIVITY AFTER 2 MONTHS ATT.
 100 % OF CASES WITH GOOD DM CONTROL WERE SPUTUM NEGATIVE WHILE ONLY 60% POORLY CONTROLLED DM SHOWED NEGATIVE SPUTUM AFB.


Discussion :

Conclusion :

DIABETIC STATUS OF TB PATIENTS HAS SIGNIFICANT INFLUENCE ON THE COURSE OF DISEASE AND OUTCOME OF TB TREATMENT.
