28 - How does Malnutrition Related Ketosis-Resistant Diabetes Mellitus differ with post alcoholic chronic pancreatitic diabetes mellitus in India.

Objective :

Malnutrition Related Diabetes Mellitus (MRDM) is not uncommon in India. This was earlier sub-classified as Protein Deficiency DM (PDDM) and Fibro-Calculus Pancreatic DM (FCPD), but were subsequently removed and positioned as secondary DM. PDDM typically come from poor socio-economic background, have negative family history of diabetes and present with very low BMI, low C-peptide & severe non-ketotic hyperglycemia. Characteristics may mimic post alcoholic pancreatitic DM.


Methods : Software-updated records of 37,600 subjects with diabetes were selected for assessment from a tertiary care center Nagpur, India. Of them, 32,569(86.6%) had Type 2 DM, 2053(5.5%) were Type 1DM, 2370(6.3%) had Gestational Diabetes Mellitus, while 601(1.6%) had Pancreatic Diabetes. Amongst 601 patients of pancreatic diabetes, 512 patient’s data was valid for analysis. Data was analyzed for comparison in two groups, Alcoholic and Nonalcoholic pancreatic diabetes.


Results :

Of 512 subjects 266(52%) had PDDM, 175(34.2%) had alcoholic pancraetitic DM without calcification, 34 (6.6%) had tropical FCPD & 37(7.2%) had alcoholic FCPD. 70% of PDDM & 67% of alcoholic DM were not on insulin for 4-5years and were either on alternative medicines of oral drugs. Discussion :

The prevalence of MRDM in India was found to be 1.6%, of them 52% were PDDM, while 34.3% were having history of alcoholism. Subjects in PDDM had lower BMI, higher blood glucose, higher microangiopathy versus alcoholic non-calcification DM.


Conclusion :

Insulin requiring ketotic-resistant  diabetes is not uncommon in India. Their insulin treatment is delayed by approximately 4-5 years, thus they are at higher risk of microvascular diseases.
