32 - Comparison of Telemedicine Screening in Endocrinology and Primary Care Populations for Diabetic Retinopathy

Objective : Telemedicine screening can identify diabetic retinopathy (DR) and diabetic macular edema (DME) in patients with diabetes. We examine the differences in efficacy of telemedicine screening of patients evaluated in either an endocrinology or primary care based setting.


Methods : This is a multicenter, retrospective chart review using the Intelligent Retinal Imaging Systems (IRIS) telemedicine platform.  For each exam demographic data, type of practice, and severity of DR/DME were evaluated. Chi-square testing was used for statistical analysis.


Results : A total of 54,361 eyes of 27,501 patients were screened. For endocrinology (13,795 patients) and primary care (13,526 patients) populations, the proportion of any DR (26.09% vs 23.19%, p <0.001) and any DME (3.96% vs 5.15%, p<0.001) were different.


Discussion : Patients screened at an endocrinologists office had a significantly greater amount of diabetic retinopathy than patients screened with their primary care physician. We postulate this is because patients were under diabetic treatment for longer or had more difficult to control disease. Surprisingly when evaluating diabetic macular edema, this finding was more prevelant in patients screened by primary care physicians. 


Conclusion : While patients screened by endocrinologist had more DR and less DME than those screened by primary care physicians. Screening was efficacious in both populations.
