22 - Correlation between cortical renal thickness and estimated glomerular filtration rate in diabetic nephropathy patients :

Objective :

Introduction: Diabetic nephropathy lead cause of chronic kidney disease is diagnosed late with uremic symptoms. This study was designed to evaluate nephropathy in early stage by measuring renal cortical thickness .


Aim: Defining correlation between renal cortical thickness and estimated glomerular filtration rate in diabetic nephropathy for diagnosing nephropathy in early stages.  


Methods :

This is a cross-sectional unicenter non interventional study conducted in 163 diabetic nephropathy patients after excluding other causes of nephropathy e.g hypertensive, obstructive, chronic progressive glomerulonephritis. eGFR was calculated by lowest creatinine within 90 days of ultrasound imaging using Cockcroft-gault formula. The renal cortical layer thickness was measured in the sagittal plane, above the medullary pyramid, perpendicularly to the capsule, in the upper and middle thirds of the kidney


Results :

Of 163 Diabetic nephropathy patients enrolled 86 (52.8%) were males 77 (47.2%) females. The mean creatinine was 1.47±0.42 mg/dl (range 0.64 - 3.20 mg/dl). 22 (13.49%)  had creatinine below 1mg/dl. Mean eGFR ( in ml/min/1.73m2 ) was 49.84±13.49 (range 41.2 -62.5). Cortical renal thickness at right upper part, right middle part, left upper part, left middle part(in mm) was 5.55 ± 1.04, 5.65 ± 1.05, 5.53 ± 1, 5.66 ± 1.02 respectively. Mean renal length right, left was 9.29 ± 0.41, 9.54 ± 0.43 cms respectively. Correlation co-efficient of Cortical Renal Thicknesses correlated better with eGFR then Cortical Renal Length in  diabetic nephropathy.

Discussion :

Measuring cortical renal thickness by regular ultrasound scanning can diagnose nephropathy in early stages when serum creatinine is in normal range


The mean correlation between CRT & eGFR was significantly higher than that of mean correlation between CRL & eGFR (p
