Is Screening of Thyroid Function Test Important? Study in an Urban City of Southern Part of India.

Objective :

Aim of the study was to establish the increase prevalence of the thyroid disease, majority of patient were not having classical symptoms and signs of thyroid diseases. We wanted to analyze whether there was increased incidence of thyroid diseases and if thyroid disorder is detected whether there is a risk of hyperlipidemia in these set of a population of Hyderabad or not.

Methods :

We have screened 1100 patients attending the OPD of which 968 patient were eligible for the study. Inclusion criteria: Patients with symptoms related to thyroid disorders and age between 15 yrs to 70 yrs. Exclusion Criteria: any preexisting illness, prior usage of Levothyroxine, Preexisting HLD, Prior CAD were excluded from the study

Results :

the data was analyzed by SPPS v 17 software, the patients were classified into groups based on their thyroid function status, there was no statistically significant difference observed in Mean age between groups p >0.05 (Table 1). whereas the 58.8 % cases were euthyroid with an incidence of hypothyroidism of 9.7 %, subclinical hypothyroidism 27.2% and hyperthyroidism 4.3% respectively.There was a female preponderance observed ratio of female: male patients was 4.6: 1(Table2 &3). 

Multiple comparison ANOVA was done to assess the difference in mean TC and TGL between different groups of thyroid disorders. mean TC and TGL was more with hypothyroidism compared to subclinical hypothyroidism, euthyroidism and hyperthyroidism p <0.05. The mean TC and TGL was significantly more in subclinical hypothyroidism compared to euthyroidism p <0.05. Correlations was done between TC, TGL and individual parameters of thyroid function observed that there was Significant and positive correlation between Total cholesterol, Triglycerides and TSH p <0.05. There was a significant and negative correlation between TGL and ft4 p <0.05

Discussion :

Ambika gopalkrishnal and etal reported a higher prevalence of thyroid disorder in cities than compared to coastal areas. The said study reported an overall prevalence of hypothyroidism among various cities as of 10.9% and Hyderabad as 8.8 %. In Our study prevalence of hypothyroidism of 9.7 %,whereas incidence of subclinical hypothyroidism was 7.9% in previous studies as compared to 27% in the current study.

Conclusion :

Thyroid disorders have no age limits; indeed, hypothyroidism is clearly more common in older than in younger adults. Despite the increased frequency of thyroid problems in older individuals, physicians need a high index of suspicion to make the diagnosis since thyroid disorders often manifest as a disorder of another system in the body. Early diagnosis and treatment remains the cornerstone of management.
