DIABETIC PAPILLOPATHY - A RARE OPHTHALMOLOGICAL COMPLICATION OF DIABETES MELLITUS

Objective : Diabetes Mellitus is a chronic disease which can lead to various complications. The most imporatant complications we know are microvascular and macrovascular complications. The common ophthalmological complication we come across is diabetic retinopathy. Here we present a rare opthalmological complication in a type 1 diabetes mellitus patient other than diabetic retinopathy.The term diabetic papillopathy is used for unilateral or bilateral optic disc swelling in which the patient has either type 1 or type 2 diabetes mellitus.This entity is diagnosed by excluding other causes of disc swelling.


Methods : A 25 year old female type 1 diabetes mellitus patient of 5 years duration came for optimisation of blood glucose. She was also complaining of blurring of vision for past few months. Clinical examination revealed reduced visual acuity (6/36 on right eye and 6/9 on left eye) and bilateral swollen optic disc without any evidence of retinopathy or signs of inflammation. Magnetic resonance imaging of brain did not show any mass lesions or features of raised intracranial tension.

Discussion : Diabetic papillopathy is a rare ocular complication, seen in both type 1 and type 2 diabetes mellitus. It is characterised by hyperemic optic disc swelling due to axonal edema and vascular leakage in and around optic disc. Exact pathogenesis is not known but include rapid metabolic control and small cup disc ratio. There is no validated therapy for this condition. It is a self limiting condition with good visual prognosis. Some authors considers this condition as an atypical form of non arteritic anterior ischemic optic neuropathy.


Conclusion : Diabetic papillopathy is a rare complication seen in diabetes, self limiting condition with  good visual prognosis and to be considered after excluding other causes of optic disc swelling.
